Why this Course:

There is no such thing as an accident – someone or something did not perform to standard. In order to prevent repeat occurrences we need to be able to identify underlying or root causes. To do so requires a thorough investigation following a methodical procedure. This course provides the framework that any investigator can take to establish sound investigation procedures for their site.


Course Organization:

The course will follow the outline below using a case study to guide the development of the principles. Participants will work in teams to investigate an occurrence in the rail yard. PPE will be required as participants will be in the rail yard conducting their investigation. Please bring safety boots, glasses, and gloves. We will provide the hi-viz vests for use during the course.


Course Outline:

Module 1 - Why Investigate?

The objective of this module is to provide the investigator with sound reasoning, costing rationale and an incident costing calculator that will assist in establishing a sound basis from which to begin the recommendation process. Topics for review: Business Case, Incident Costs, Regulations


Module 2 - Be Prepared for the investigation

The objective of this module is to assist the investigator in preparing to commence an investigation and to ensure that the investigator has the necessary equipment to successfully complete the investigation. Topics for review: Definitions, Company Culture, Investigator Perimeters, Investigator Response Equipment


Module 3 - Site Securement

This module will teach the investigator the importance of securing the fragile and necessary information that is available at the incident scene before normal operations are resumed. Topics for review: advance site securement and on site securement.


Module 4 - Gathering of Facts and Data

This module will teach the investigator the major areas from which to gather facts and data pertinent to the investigation. In addition, it will provide an introductory skill sets to assist in collecting information in a manner relative to the investigation process such as documentation, photography, interviewing and event plotting.


Module 5 – Analysis

The objective here is to provide the investigator a process to follow where the information collected from the investigation can be formulated to present a factual report with supporting documentation for the investigative results. Topics for review: identify events, types of analysis, type of barriers, preventive options and definitions.


Module 6 - Identifying Cause

This module will provide the investigator with different areas to focus upon when identifying the root cause of the incident. Topics for review: Management control, supervisory and personal factors.


Module 7 - Developing Recommendations

This will provide the investigators a format to evaluate their pending recommendations to prevent recurrence to improve their success in The Canadian Heartland Training Railway Page 3 implementing preventive measures. Topics for review: evaluation, feasibility, added risks, and implementation.


Module 8 – Communication & Follow up

This is the most important phase of the entire investigative process because if the investigator cannot communicate to induce action to change then the investigation has not achieved the necessary results to prevent recurrence. This module will provide the investigator with a process from which to develop effective communication. Topics for review: change agents, action agents, types of communication and the importance of the final written report. The final piece of a successful investigation is the follow – up to ensure the agreed to preventive actions have been or are being implemented.


The Course Leader: Art Nordholm

Art has over 30 years of leadership; investigative; organizational; business management; problem solving and professional safety management experience. He is a Canadian Registered Safety Professional (CRSP) and a Certified Health and Safety Consultant (CHSC). His skill set assists organizations and corporations to mitigate risk and improve their safety systems to recover financial losses from identified safety deficiencies. His career has helped him acquire extensive knowledge and skills applicable to all industries. In addition, managerial and problem solving skills gained from a first line supervisor's role to a senior management role provide a solid foundation for applying professional "best practices" in OH&S management techniques. His innovative, decisive, pro-active approach to solving safety and management problems has proven effective in achieving solutions to OH&S problems and concerns.

Art is one of four auditors in the world qualified as a CMS Lead Auditor Tutor/Instructor for Smart-Iso.

Currently Art is the President of the Canadian Society of Engineering (CSSE). He has also held the position of Vice President, Executive Secretary and National Chairperson for North American Occupational Safety and Health Week (NAOSH) for Canada.

In addition, he teaches Incident Investigation for the Facility of Extension at the University of Alberta.
